Hospital stays often result in new prescriptions, altered dosages, or discontinued medications.
This 1:1 meeting reviews current hospital discharge orders to ensure medication plan is safe, accurate, and fully understood. This medication reconciliation prevents dangerous drug interactions, clarifies dosage changes, and lowers the risk of unplanned hospital readmissions. Compliance packaging is a great option post-hospital discharge.
